Exploring the way structural factors in services influences change and delivery of trauma informed approaches
The things that services choose to deliver and the way the services are organised and commissioned. Trauma informed services deliver a range of interventions aimed at healing and growth and are organised collectively to bridge all the needs of the population seamlessly.
The two main structure standards are Healing Interventions and Responsive System Design.

Interventions that are trauma-informed, and any support that is delivered to be done so in a way that appreciates the impact of trauma and minimises further harm.

People in the whole system can easily access a range of therapies, which are specially designed to treat trauma, for the length of time that is right for them.

Developing and embedding a bespoke Trauma Informed Care clinical model
As with most forensic services our care delivery was largely informed by the medical model and related infra-structure such as the Mental Health Act and Ministry of Justice. There was increasing frustration that this model failed to match the complexity of our service users and the knowledge staff held around the lifelong impact of trauma.

The treatment lends itself to filial therapy, attachment and relational health, impaired neurosequential development through to cortical modulation and beyond throughout adulthood.

A participatory action research approach was used to engage with both the women in prison and the staff to understand the context, function and possible alternative to self-harm.
